The form is set up, but all of the input field error messages display. I corrected this after adding a call to the form's style sheet. But, the style sheet causes a conflict with my page's right column, causing any text field to expand outside the block, as well as increasing the font size dramatically.
After submit, I get a 404 error. Admin panel does have thankyou.html set, and the file does exist.

Solved.
Adjusted the style sheet. No conflicts. In admin., appended the correct path to the redirect. Everything works.
Two comments:
Since we have focus effects, we can also do the same with text. What is now the "label" can reside in the text input field. Upon a change, the text will disappear for the user to enter their input. If the user leaves it blank and changes to another input field, the text would re-appear. I did this with client-side JavaScript, but your form is built on-the-fly, and I'm not well versed enough in php to effect this.
Perhaps a consideration to add, or for a future build.
The form checks for an empty field in the "comments", but I can still sunmit it by entering a space character. Can we not test for this?
